PRIOR TO CHECK IN:
Please visit this website and submit a final roster to the Club. Do this if you have added or dropped players since first registering. If you have not made any changes, simply select "no changes". This is important so we can make sure that our official tournament roster and game changes include all your players. DEADLINE: 8 a.m. Thursday, May 17. DURING CHECK IN: Teams should check in for Kohl's Cup a minimum 1 hour before your first tournament game. Please pay your $10/player fee at that time. Checks should be made out to Columbia Soccer Club. Check in will be conducted at tournament headquarters tent directly south of Cosmo concessions stand If possible, teams should check in as a group, as opposed to individual players checking in on their own. However, players who cannot check in with your team should report to the tent for individual check-in prior to play. Each player must do the following three things before being allowed to play: 1) Turn in a signed, notarized MYSA Medical Release and Liability form (available here for download) 2) Provide a small picture for use on their player identification card. Picture should be roughly postage stamp size... similar to 3) Sign player identification card and team roster. Players who have completed these three steps will be issued a laminated player i.d. card (which should remain in the coaches possession for the tournament) and will be allowed to play. Referees will be instructed to not let players without i.d. cards participate. Coaches -- please ask your team to meet you at the tournament headquarters tent for check in, and when all players have arrived, bring your entire team up for the process. We suggest you do this at least one hour prior to your first tournament game. We also ask that you line your players up alphabetically, to speed up the process. Each player will present their medical release and photo. Then they will sign their player card and team roster. The club will laminate your team's player cards and deliver them to you prior to your first game. Players who are not available for team check-in should be asked to visit the tent to check in as individuals. They will bring their player card to you. Player cards should be kept in your possession for the tournament. You will be given a packet of medical release forms after check in. You should keep them with you during all games, in the event that medical assistance is required. After the Cup is over, you can give the player cards to your players as a memento of the tournament. |
