CHECK-IN PROCEDURE FOR 2007 Pride Spring Invitational

ALL TEAMS SHOULD CHECK IN A MINIMUM 1 HOUR PRIOR TO THEIR FIRST GAME.

FRIDAY, APRIL 20 -- 5 p.m. - 11 p.m. at SoccerPro, located north of the Columbia Mall on Bernadette.

Directions from I-70
Exit Stadium Drive
Turn south towards the Columbia Mall.
Go 2 blocks. Turn right on Bernadette Drive (at the Drury Inn).
SoccerPro is on the right in Bernadette Square.

SATURDAY, APRIL 21 -- 6:30 a.m. - 2 p.m. Tournament Headquarters, located just south of the concessions stand at Cosmo Park.

ITEMS WE NEED PRIOR TO CHECK-IN:

All teams should fill out an official Columbia Pride Invitational online numerical roster. Doing so will speed up the check-in process. Changes to this roster can be made when you physically check in, but we ask that every team submit a numerical roster online by NOON, THURSDAY, APRIL 19.

No roster additions will be allowed once a team begins tournament play.

NEW FOR 2007 SPRING PRIDE INVITATIONAL. To expedite check-in, please FAX a copy of your signed official USYSA state roster (MYSA roster, Illinois roster, Kansas roster, etc.) to 573-446-9525 no later than NOON, THURSDAY, APRIL 19.

ALL TEAMS SHOULD BRING THE FOLLOWING ITEMS TO CHECK-IN:

Items we need to review:

1. State Association Issued Player Identification Cards (MYSA, KSYSA, IYSA, OSA, etc.)
[important announcement US Club or other organizations not affiliated with USYSA]

2. State Association Medical Release Forms.
Missouri Youth Soccer Medical Release Forms must be NOTARIZED. Missouri players without notarized MYSA medical forms will not be allowed to play. Out of state teams shoud follow individual state association rules regarding medical release forms.

Items we need to keep:

3. A copy of team's signed USYSA-sanctioned official state roster (for instance, MYSA roster for Missouri teams).

4. Official state association travel permit for non-Missouri teams.

5. Signed guest player permission form (if applicable).

6. If applicable, proof of grade for 8th grade/U15 boys playing on a U14 boys team (up to two allowed), or Senior/U19 girl playing on a U18 girls team (up to two allowed). Proof of grade could include copy of grade card, copy of student i.d., letter from school official attesting to player's enrollment, etc.
