CHECK-IN PROCEDURE FOR THE Fall 2008 COLUMBIA INVITATIONAL hosted by the Columbia Soccer Club -- Columbia Pride

ALL TEAMS SHOULD CHECK IN AT LEAST 30 MINUTES PRIOR TO THEIR FIRST GAME.

Check-in Location:

SATURDAY, October 18 – beginning at 8:00 a.m. at the Tournament Headquarters, located just south of the concession stand at Cosmo Park.

ALL TEAMS SHOULD BRING THE FOLLOWING ITEMS TO CHECK-IN:

Items we need to review:

1. State Association Issued Player Identification Cards

2. State Association Medical Release Forms
Missouri Youth Soccer Medical Release Forms must be NOTARIZED. Missouri players without notarized MYSA medical forms will not be allowed to play. Out of state teams should follow individual state association rules regarding medical release forms.

Please have the above items in alphabetical order.

Items we need to keep:

3. A copy of team's signed USYSA-sanctioned official state roster

4. Official state association travel permit for non-Missouri teams.

5. Signed guest player permission form (if applicable).

6. If applicable proof of grade for 8th grade/U15 boy playing on a U14 boys team (up to two allowed), or Senior/U19 boy playing on a U18 boys team (up to two allowed). Proof of grade could include copy of grade card, copy of student i.d., letter from school official attesting to player's enrollment, etc.
